i need to reproduce payment check in LaTeX for Croatian standardized Payment form like this:

Anyone has some guide or working form how to reproduce this as a table in LaTeX ?

Note: It doesn't have to be colorized.

I've never done such task and my comment might not be true. I think one choice is a table however it would be very tedious task.
