You can use hyperref to set metadata for the generated PDF (e.g., the pdfauthor or pdfcreator fields), but that's indeed more of a question for TeX.SE. Whether anybody actually extracts that metadata in a structured fashion so that there's any benefit from doing this is a different matter...

I think this has to do with the new security features in OS X that limit even root's ability to modify files under /usr. It's a big change in OS X 11, but already started with the 10.10.5 upgrade, which resets the permissions on /usr/local to read-only (and in particular, removes the x bit for directories). You could try a judicious chown -R $USER:admin /usr/local/texlive (where $USER is you username) and retry (without sudo).

Are you asking because you want your self-compiled PDF to look like a SIAM paper, or because you want to include TikZ figures in the TeX sent to SIAM for production of the published version?

@tgkolda: Excellent news! The problem could just be a missing tocdepth counter -- try adding the line setcounter{tocdepth}{3} somewhere in the class file.

Hard to say, since the fontsize mechanism runs pretty deep. I had limited success with the first option, but the second (fixing the scrsize files) worked for me.
