The source can be downloaded from the writelatex link @samcarter provided in his comment (click on menu in the top bar, project, download as zip). The font is not included, though (but frankly I find it too hard to read for any (half-)serious presentation anyway). You could try one of the fonts from fontspace.com/category/blackboard with xelatex or lualatex.

This was likely due to some backward-incompatible changes to the inconsolata package, which has since been sorted out - the latest version on CTAN should be compatible again. Can you check if updating MikTeX now fixes the problem?

You can combine this with the newtx math fonts: \usepackage[libertine,cmintegrals,cmbraces,vvarbb]{newtxmath}, and (if necessary) inconsolata for the monospace font: \usepackage[scaled=0.95]{inconsolata}.

You need to combine it with a customization of the author field; see tex.stackexchange.com/questions/12806 for an introduction and tex.stackexchange.com/questions/6743 for swapping fields. (I'm not expert enough to whip up biblatex solution, but I have one using bibtex if that is of any help?)
