Yes, thanks for the help. I still think this answer is a bit far from being a complete solution; but at least it points to “a direction” of how to implement this. I'll award you the bounty as there is no other answer. Will try to work more on it myself and see if I can eventually produce something more robust, perhaps suitable for publishing as a package. This is looking very good! A problem I've found, though, is that the first use of an \item within an itemize won't be able to read any optional arguments, e.g. I can't write \item<+-> for overlays.

Nice! Although for key=value items I would mention \keyval_parse:NNn instead, as this is the actual function doing just the parsing (instead of the more high-level option/key handling).
