Frank, thank your for your great answer. Plan A did the trick. The appendix chapter was at the very end of the TOC so need to turn it on again. I was looking a solution for so long :)

Thanks!, yeap no need to be title at all just "References (Continued)" at the very top of the last three pages in references.

Thanx for this answer, it helped me to modify the toc title, but I couldn't managed to add the "Continued" if TOC spans over 3 pages. It shows up on the second page but not on the third. Could you help me with this?
