I forgot I had defined `and` as `a\.` which means I need e.g. `{c e}` not `{ac e}` in the text composite declarations.

Thank you belatedly for the effort that went into this answer. This is neater than the hack that I've been deploying, namely, of using hyphen to separate chapter and example numbers [e.g., (2-1)], letting makeidx compile that (it doesn't object), then opening up the .idn file and doing a search and replace of hyphen for dot.

comment Index shouldn't start new page
One facet of this solution (which is otherwise very nice) is that, on the opening page of the index, the righthand column starts at the top of the page. I think it would look neater if the top entry of the right column were aligned with the top entry of the left column. Is that possible? Thanks. (Sorry I can't post a pic in this comment.)
