Thanks for your answer. The first one was on the right track. \rhead{\rightmark} makes the heading mark appear to the right even when its a page with no headers (e.g. at the start of contents), however \lhead{} alone did what I needed.

Actually I just noticed this approach as a problem because the next row are A B C A B C but you changed the CSV data to have unique names instead - the table renders as " A B C A-index4 B-index5 ... "

Sorry, I cannot, not being a ConTeXt user - I took what was in the wiki article at face value. Perhaps you could expand your answer to debunk this, and maybe even give a minimal thesis template example in ConTeXt?
