Not related to the problem, but you don't need to load graphics (just graphicx), there is no point loading both color and xcolor, and there is no need to load packages more than once. In any case, do you actually need all those pacakges?

Your example works OK for me. What distribution are you using? Where did you obtain mcode.sty? Not related to the problem, but I really don't think you want \input epsf.

Sorry, couldn't help it. Seriously though, it's not very difficult to use LaTeX without LyX, and doing so will probably save you a lot of time in the long run.

You need to use an 'at' symbol to notify another user in a comment (e.g. @DavidCarlisle). Anyway, I think your problem is down to conflicting definitions of pixel size, and can be resolved by using points, rather than pixels (see my answer below for more details).
