Nevertheless there is something strange about @wilx's example code. As he says, removing apparently harmless lines of code eliminates the problem.

I think you would be better off posting an example code that reproduces the problem. It is likely that the error is caused by a problem with your document, rather than an outdated package.

@DVK --- you are right; that's the documented behaviour. Perhaps columnwise changes are an undocumented feature that works with some documents but not others. Does my example work on your machine?

@DKS --- I have no explanation for this. Page 4 of the lineno package documentation says that the switch option has this effect. It worked when I posted the answer and it works on my machine now (TeXLive 2014). Perhaps you could ask a new question about the issue, including the .log file generated by the example.

Do you actually need tcilatex? As @egreg points out, much of it seems to be definitions of macros from amsmath. I suggest commenting out \input{tcilatex}. Then copy and paste any parts that are required into the preamble of your document.

@JuriRobl --- I've tried TeXLive 2012 and TeXLive 2014. In both cases pages 23 and 24 are blank if \include is used. With \input the problem does not occur. Some classes/packages might be smart enough to deal with the new page created by \include, but it's better to use \input in most cases.

If removing new material does not work, try deleting all files generated by TeX (.aux, etc.).
