@DKS --- I have no explanation for this. Page 4 of the lineno package documentation says that the switch option has this effect. It worked when I posted the answer and it works on my machine now (TeXLive 2014). Perhaps you could ask a new question about the issue, including the .log file generated by the example.

Do you actually need tcilatex? As @egreg points out, much of it seems to be definitions of macros from amsmath. I suggest commenting out \input{tcilatex}. Then copy and paste any parts that are required into the preamble of your document.

@JuriRobl --- I've tried TeXLive 2012 and TeXLive 2014. In both cases pages 23 and 24 are blank if \include is used. With \input the problem does not occur. Some classes/packages might be smart enough to deal with the new page created by \include, but it's better to use \input in most cases.

I can reproduce the problem on a machine with TeXLive 2012. I'm not sure what the cause is, beyond the fact that the problem seems to occur at the latex stage; it's not an issue with dvips or ps2pdf as I originally thought. I'm fairly sure that updating will resolve the issue.

You could try without the -Pdownload35 option, or check the configuration for that printer. Another possibility would be to update your TeX distribution. Beyond that I don't know what to suggest, because I still can't reproduce the problem.
