The nice thing about this answer is that \vtop{\hbox{\strut top line}\hbox{\strut botline}} can be used within Pandoc Markdown tables.

@StevenHouben That is exactly what I have been warning for in my previous comments. The problem has no ready solution in LaTeX, nor ConTeXt (see linked questions in the SE margin). This led to my now famous question: Is LaTeX intended for unattended typesetting? In the end, I solved this problem through unattended CSS typesetting with PrinceXML. Here is an example with Pandoc Markdown source & makefile. Good luck!
