You could try the ifthen package and define a new environment that does nothing whenever you want all slides and is \comment when you want to exclude some. You just put this environment around the slides in question.

The problem here is hyperref, it can only handle text. If you create a titlepage in your document, the title looks fine. If you try using your title as a section title in article, the navigation will be messed up as well. I'm not aware of any way to trick hyperref into doing non-text right...

Why would you want to have it as a superscript? That would look really weird IMHO... A footnote is a superscript, but the reference is just the number. If, however, you don't want to refer to the footnote, but reuse it, you could just use $^{\ref{footnote 1}}$, which looks like really bad tex code, but works...

What is the problem exactly? The referencing seems to work fine, but you should write London is a city, cf. footnote \ref{footnote 1}, then it won't look quite as awkward.

@Nek So you want a template to produce a book (or whatever) that looks like those of O'Reilly? Then why don't you ask the publisher? If they don't give it to you directly, I don't think you will be able to get hold of it legally...

I didn't realize pstricks works at all with pdflatex. I haven't used it, but have you considered tikz? If you have a lot of pstricks stuff, this might be too much work, but maybe it is possible to combine them? Like, use your plots, but create the arrow with tikz?
