But it does seem that under the first displayed equation, I'm getting one below-displayed-math space plus one above/below-theorem space. In other words, it seems that ntheorem is removing the space after the proof but leaving the space before the theorem, when it should also be removing the space before the theorem.

Try loading subfigure with the tight option: \usepackage[tight]{subfigure}. Does that reduce the space?

I was just copying what they did in the upref package; they say that \sw@slant gives you an error if it comes at the start of a paragraph and at some other locations.

Could this somehow be fixed directly in the \crefdefaultlabelformat command? I'm guessing that, in general, there could be other commands I'd have to change besides \crefpairconjunction.

You can first "undefine" the command by \letting it be equal to \relax, e.g., \let\div\relax \DeclareMathOperator{\div}{div}.
