@HendrikVogt: mostly the source code is the documentation. MS has released a paper about OpenType math typesetting, but that's far from complete, so some parts are just reverse-engineering whatever Word does.

Generally speaking, \ifx only looks one level deep: it checks whether all tokens have the same indices into the equivalence table, but does not check whether they have the same meaning. The same works with \let\foo=\relax: Now \foo is a token with the meaning of \relax but a different name.

On the other hand, \expandafter\ifx\x\relax is true. This works for all primitives that have a frozen version and therefore appear twice in the equivalence table. With LuaTeX, you can create additional copies of primitives using tex.enableprimitives, and they show the same behavior.

AFAIK setting the extended font dimensions is ineffective in XeTeX (they don't actually get applied), so you can do this only in LuaTeX (\Umathsupshiftup=12pt).

Note that LaTeX floats don't use TeX inserts. LaTeX uses inserts only for footnotes and \enlargethispage, so you can always take the necessary allocations from the extended pool no matter what \insert accepts. See latex-community.org/forum/viewtopic.php?f=5&t=3049 for an example.

@Martin: No, the second " doesn't have to be active; there is no such restriction on non-parameter tokens in parameter texts. A mathcode of 8000h means math-active: in math mode, the character is treated as if it were active, and TeX uses its meaning.

Also, there should be no space between cos and the opening parenthesis. I think this is one of the examples where replacing all delimiters by auto-scaled ones doesn't work reliably.
