Useful notes for some of my answers:

CTAN lion drawing by Duane Bibby.

minimal working example](http://www.minimalbeispiel.de/mini-en.html)

The demo option for graphicx simply replaces actual figures with black rectangles; do not use that option in your actual document.

Then the problem is elsewhere and the code you posted is not enough to detect and diagnose the problem. Please compose a minimal example really illustrating the problem mentioned and edit your question, adding this new code (don't forget to post a complete compilable document).

@giannis I edited your code and added the missing packages. Please, using the exact same code that appears in your question (after my edit), are you still getting the mnetioned problem?

@azetina new update. Thanks to an optional argument, you can now control the position of the box and of the pointer.

comment Have \underbrace lead to an arrow pointing towards expression (not replace underbrace)
@dougphy as I said, the problem is not errors. \d is an already defined command to produce a special accent. With your definition for \d you are overwriting an existing command. I suggest you to not do that, anc choose another name for your command.

@jafan this library was developed by user Luigi from his answer to one of the questions you linked to. The library is still not part of PGF. You might have to ask the author why is this so.

comment Have \underbrace lead to an arrow pointing towards expression (not replace underbrace)
@karlkoeller sorry; I thought you had \newcommand\d{...}. Using \nenewcommand\d{...} doesn't produce errors, but it's not correct, since \d is a basic command to produce an accent (a little dot below the argument).

comment Have \underbrace lead to an arrow pointing towards expression (not replace underbrace)
@karlkoeller probably, yes, except for \renewcommand\d{...}; he must have \def\d{...}; otherwise, an error is triggered.
