Off topic? Okay. I asked it here because it was the TeX that wasn't converting (the Markdown is actually irrelevant to the question). But for posterity, the problem turned out to be the particular font - Cambria - which, despite this being a clean install of Office, seems to be missing or broken on my system. Changing the font to XITS Math fixes the math display, so Pandoc is probably off the hook here.

How? I tried with bibentry and natbib; what I got was the "actually looks like" version above. The document in my question is just a minimal version of the actual document, exactly as it would appear.

Thanks, Guido! I had been meaning to try biblatex, and that was a great introduction. I managed to simplify it a bit by using \fullcite to enumerate the key references (they don't appear in the main text), but I learned a lot from your answer!

Damnit, of course as soon as I post this, I find part of the answer; using \textwidth instead of \FBwidth solves the shifting problem. The docs even mention this explicitly by saying that it has to fit in the \hbox. I still don't know what's up with the table numbering, though. When I change the .tex to \textwidth, the tables are numbered 2 and 5!
