@Joseph only lower case letters matter, everything else can be passed through, no expansion. AöT\hello{\hello} contains one small letter thus A\texttt{ö}T\hello{\hello} is the desired result. EghF\texttt{s}D contains three but the third is hidden in the group token, so the desired result is E\texttt{g}\texttt{h}F\texttt{s}D. Just processing the first level, no expansion.

I was activley avoiding a regex solution. It always reminds me of the famous quote: Some people, when confronted with a problem, think “I know, I'll use regular expressions.” Now they have two problems. From a computer science point of view: looping over a linear list, inspecting each element and performing an operation on it, is such a simple task that using regex feels like overkill. But thanks I will look into it.

@egreg, lualatex and utf-8, the last call has one token, that is not a letter only on command sequence, it should go through untouched.

What 'original TeX' refers to needs to be clarified as well. The first TeX was written in SAIL to run on a PDP-10 under the WAITS OS.

Do you use Unicode for your documents? Then the ASCII 022 Quotation Mark " is just one of many options you have. See here. Then how to enter Unicode characters with Emacs is worth a look. Regardless of the encoding the packages csquotes might also be worth a look.

@JLDiaz With LuaTeX 0.76 from TeXLive 2013 I get: LuaTeX error DetectUnderfull.lua:8: You cannot set field cmd in a node of type whatsit. Does it still work for you?

Are the entries in column 1 and 2 dependent or independent? That is, does an entry in column 1 summarize what is written in the three rows it spans or does it give a category for the three rows it spans? Is it always three or does it vary? If it varies is there are rule how it varies or perhaps an external file that contains the data?

I’d like to know where the “missing character” log entries are from? If I look into LuaTeX’s Transcript written on test.log. I cannot find them there. If I remove \coloneq and the other macros from the MWE and just use \[:=:=\] I still get the same errors. As I’m only working on the preamble and not on a true document Latin Modern Math was just an unfortunated random pick it seems. From the few available OpenMath OpenType Fonts Neo Euler seems to be the only one to have a non-symmetrical :=.

@Josh The pdf of the nature article you are referring to, hides behind a paywall. Only people surfing from inside an university network are probably able to see it.
