This post is building from the post found here, where the longtable environment is added. Can you tell me how to get this code to work properly, i.e. for the table to break properly over two or more pages? Thanks.

You have several errors in your code. For example in longtable you have to write a \\ after \caption and \label.

Then does longtableoffers an firsthead of the table and following heads of the table, same with the foot of the table.

So at last please read the documentation of longtable (try texdoc longtable on your console/terminal) and study my complex changes in your code (marked in the following MWE with % <=======================================):
