For unnumbered chapters, you can add this code to your preamble:

\titleformat{name=\chapter, numberless}[display]{\filcenter\Huge\bfseries}% 
{\titlerule[5pt]}{-2.2ex}{\hrule\vspace{1.5ex}}[\vspace{0.75ex}\endgraf\hrule]
