bibtex: hyperref doi

Question

I would like to change the abbrv.bst file such that hyperlink to DOI is given tojourunal name-#(##), pp ##-##

such that it will look something like \href{http://doi/}{jourunal name-#(##), pp ##-##}

How can I achieve this?

I need to modify somewhere around here, but I'm not sure how to...
